Who We Are
Hiawatha Care Center promotes health, growth, and rehabilitation of our residents to reflect their highest functional levels. We believe in maintaining individual dignity, and we affirm that all individuals have rights and needs that must be respected. Hiawatha Care Center will strive in every way possible to ensure the safety, comfort, and happiness of our residents. We appreciate and encourage family and friends to remain involved in a resident’s life.
Nursing
Hiawatha Care Center provides 24-hour professional nursing care. The care provided is given by qualified, experienced nurses and nursing assistants as well as the social worker, dietician, activity director, consulting pharmacist, and consulting physical, occupational, and speech therapists. We have an outstanding medical director. Each resident's care is individualized and planned to help the resident achieve his/her maximum potential in a supportive, caring environment.
Activities
Our activity program is designed to encourage maximum participation for all residents. All residents are encouraged to attend the activities of their choice. For those residents who choose not to attend, we provide individualized activities suited to their needs and interests. Ideas are welcome as we strive to make the activities interesting, challenging, peaceful, and current. Activities outside of the care center are also provided. A monthly newsletter along with a calendar of events is given to each resident and mailed to family members.
Social Services
Our social service team strives to provide support to residents and their families throughout their entire experience at Hiawatha Care Center by ensuring all psychosocial, mental, and emotional needs are met. Our team provides assistance navigating the transition to our facility, offering support throughout the entire length of stay at Hiawatha Care Center, as well as helping to access resources and services within the community as needed. In addition, our social service team will advocate for our residents and make certain they feel dignified, respected, and safe.
